An unidentified person dental report form should be given to the coroner/medical examiner or forensic odontologist. The dentist makes descriptive entries regarding each tooth using the appropriate codes. Restorations are categorized based on the side of the tooth that is affected. These five sides are:
• Mesial-side of tooth towards the midline of the mouth
• Distal-side of tooth away from the midline of the mouth
• Occlusal-Biting surface of tooth
• Facial-surface towards lips and check
• Lingual-surface towards the tongue
