Motor Vehicle Traffic Fatality Reporting

Agencies handling or investigation traffic fatality accidents should, immediately upon completion of the initial report, direct a message to mnemonic MVFR (Motor Vehicle Fatality Report) with the following information:

1.  Date and time of accident

2.  Name(s) and date(s) of birth of person(s) killed

3.  Date and time of death

4.  May names be released to press?

5.  Location of accident (city, town, street, county road, etc.)

6.  Type of accident (one/two car, cycle, etc.)

7.  Number and types of units involved

8.  Position of person(s) killed (driver, passenger, front right, etc.)

9.  Investigating agency

10. Specific safety equipment used

MVFR is a “receive only” mnemonic.  Any questions should be directed to terminal WOLN.
