The Crime Information Bureau furnishes contributors with fingerprint cards, death notice forms, final disposition report forms, and postage paid addressed envelopes upon request.
The CIB preprints the fingerprint cards with the TIME jurisdiction number, name and address of the contributing agency. Fingerprint cards should be ordered before the contributor's supply is depleted to allow time for the printing. A minimum of three months' requirement should be ordered.
Each shipment of supplies will contain a "Supply Request/Order Form" DJ-LE-235 for future orders. This form may also be found at http://www.doj.state.wi.us/dles/cib/forms.
