The Final Disposition Report (DJ-LE-249) is a single copy white form, 8" x 8", with green colored printing. The left side of the Final Disposition Report provides blocks for descriptive information of the individual, the Contributing/Arresting Agency, their OCA number, State Identification Number and arrest data. The right side provides space for action taken by the Prosecutor or Clerk of Court, along with the Court Case Number. This includes Findings & Sentence information or any additional information that explains actions taken by the court.
Only final disposition forms containing a field for the ATN (Arrest Tracking Number) are to be used. All other disposition forms should be destroyed.
