Law enforcement agencies that are required to collect and submit DNA samples to the Milwaukee State Crime Laboratory are asked to contribute a complete ten print fingerprint card of the individual. This procedure ensures all DNA registrants within the state database will also be recorded at the Crime Information Bureau.
The fingerprint card should include the rolled images of all ten fingers, as well as the plain impressions. Additional required data includes:
1. THE NAME OF THE REGISTRANT
2. THE STATE IDENTIFICATION NUMBER
3. THE MONTH, DAY AND YEAR OF BIRTH
4. SEX/RACE
5. HEIGHT/WEIGHT
6. EYE AND HAIR COLOR
7. THE DATE OF THE REGISTRATION (RECORD IN THE DATE OF ARREST BLOCK)
8. THE CONTRIBUTING AGENCY
9. THE PLACE AND COUNTRY OF CITIZENSHIP
Finally, the contributor should record “DNA Registration” within the charge field.
