This form is used by law enforcement agencies to notify the CIB that an individual whose fingerprints are on file, is deceased. Upon receipt of this notice the CIB properly annotates the criminal history database to reflect this information.
Agencies who become aware of the death of any person having a State Identification Number (SID) must complete the form and send it to the CIB. Accuracy is essential and the SID must be included. If possible, an inked rolled fingerprint impression from the deceased can be placed on the form to provide positive identification.
Agencies investigating a death should obtain all ten inked impressions on a regular fingerprint card and the death notification should be forwarded to the CIB.
In cases where an unidentified corpse is found, all ten inked impressions must be obtained on a fingerprint card and submitted to the CIB, where a technical search will be made in an effort to determine an identification.
