Date of Birth

REQUIRED  This space is to record the date of birth of the individual whose fingerprint impressions appear on the Final Disposition Report.  The order of this data must be; Month (01 thru 12), Day (01 thru 31), and Year (00 thru 99).  THE DATE OF BIRTH MUST BE INDICATED AND MUST BE NUMBER FOR NUMBER EXACTLY AS IT WAS RECORDED ON THE RELATED FINGERPRINT CARD SUBMITTED TO THE CIB.
