Printing Help: Some printers do not automatically scale the document and will cut off the bottom of the document’s pages. Your application must:
- Be completely filled out
- Be signed in all areas indicated, to include the statement that the information provided in the application and any document submitted with the application is true and complete to the best of the applicant’s knowledge.
- Include a check made out to the Wisconsin Department of Justice in the amount of $50.
- Include a copy of your proof of training.

Name Changes: No later than 30 days after a legal name change, a licensee shall provide the department with written notification of the name change and shall apply for a license under the new name utilizing DOJ form DJ-LE-293. The cost for a name change is $25 ($13 for a new background check and $12 for a replacement license).
Address Changes: IMPORTANT
Please ensure that you have notified the Department of Transportation (DOT) of any address change as required by law. The current residential address that you list on your concealed carry application MUST be the same as the one registered with DOT or your application will be denied. You can make address changes with DOT online at: http://www.dot.state.wi.us/drivers/drivers/address-change.htm
Future Address Changes or Lost Licenses:
Use form DJ-LE-285 to report a change of address or to request a replacement license.
What to do if your application was DENIED
If the review of your application or subsequent background check indicated a potential prohibiting offense, your application was returned as a denial. As explained in the FAQ’s and the emergency rules, you have the option to appeal this decision to the Department of Justice. The emergency rules establish specific timelines for the appeal process and explain what documentation you will need to include in addition to the DOJ appeal form.
