Caregiver Information

Caregiver entities required to do background checks under Wis. s. 50.065, and child care entities under s.48.685 may request both an inquiry into criminal files at CIB, and into other files at the Wisconsin Department of Health Services and the Wisconsin Department of Regulation and Licensing.

Requests are automatically forwarded to DHFS for processing when you select request-processing type "caregiver" or "daycare". Your return address you supply will be used by DHS to mail their response to you.

"General Information" requests will not be forwarded to DHS. "General Information" requests will not meet the requirements of Wisconsin statutes for caregiver background checks.

A "daycare" type request differs from a "caregiver" request because an additional file is searched at CIB. This additional file contains records of juveniles who have been adjudicated delinquent of crimes that may disqualify them from daycare employment.

The DHS IBIS letter is provided as a link for online requests. If the record request is requested by paper form, the letter is mailed and will arrive separately from the criminal response.

• Noncredentialed Caregiver Findings of Abuse or Neglect of a Client or Misappropriation of a Client's Property
• Denials or Revocations of Operating Licenses for Adult Programs
• Denials or Revocations of Operating Licenses for Child Programs
• Rehabilitation Review Findings
• Status of Professional Credentials, Licenses or Certifications maintained by the Department of Regulation and Licensing (please ignore credential findings on anyone other than the person for whom you requested a background check).

Each "caregiver" or "daycare" request creates and additional charge of $3 for DHS for processing of the request. This fee must be included with your request if you submit payment with your requests. It will be charged to your account if you are billed monthly by CIB. All payments should be made to CIB, and CIB will forward the $3 fee to DHS.
